The Licensed Teacher Report form merged into the Credentialed Leaders Form and is to be completed annually by March 31. Unity Worldwide Ministries requires that each licensed teacher, minister and spiritual leader submit this form.
The link to our new online Credentialed Leaders form was sent to leaders in January with reminders in February and March, to the main email addresses we have on file for each leader. Please look for this email from Unity Worldwide Ministries with the subject line "2022 Credentialed Leaders Annual Report Instruction and Link". If you cannot find the email with the link in your inbox, spam, or promotions folders, please use the Credentialed Leader Annual Report Form in Word below and send to AnnualReportForms@unity.org.
Note: You may wish to confirm your correct email address with our office. LUT sponsors will be notified in April that they were listed as a sponsor. If an LUT doesn't have a minister, their regional rep is their backup sponsor.
